I give permission for my child to attend this EBS enrichment program and give permission for emergency medical care to be provided by the nearest medical facility.

I understand it is my responsibility to arrange adult supervision for my child immediately after class and that I may be charged an additional $20 per half hour for supervision starting 10 minutes after the class has ended.

Non-Internet Photos: I give my permission for my student to be photographed for non-internet purpose including, but not limited to: school yearbook, bulletin boards, class projects, etc.

Internet Photos: I give my permission for my student to be photographed for school purposes including, but not limited to, school website, classroom websites, etc. Individual student names will not be posted.

News Media: I give permission for my student to be photographed or videotaped by the news media.
